Course Schedules
LifeLink Comprehensive Course Schedule
Revised
08/28/2008
OHIOHEALTH ASSOCIATES:
Starting July 1,
all Basic Life Support (BLS) and Advanced Cardiac Life Support
(ACLS) courses will transition to a self-directed online
and independent lab
training program developed by the American Heart
Association.
If your
BLS or ACLS card is due for renewal
soon, you may have
already been assigned
one of these courses. Log on to the
OhioHealth
University to find out. You may also
contact Sarah Jones at
566-2762 or
Brenda Stanton at 566-2764 for
more information.
Course Fees and Registration
All classes require
pre-registration, payment and written enrollment confirmation. Read
below to learn "How to Register For A Class."
HOW TO
REGISTER FOR A CLASS:
All students
must submit a completed registration form and make payment for a class before
their enrollment can be processed
and
confirmed.
We suggest that you
complete the registration form, then fax it to us at (614)
566-8077. Immediately after faxing, call the
LifeLink office at (614) 566-9019 to confirm our receipt
of your registration form and that the class date you requested is available.
You can then pay for your class with a credit or debit card by phone. See the
applicable
course registration form for current course
fees.
Once we receive the student's completed registration form, confirm
that the student's requested class date has student openings, and
payment has been made, LifeLink will
issue the student a written enrollment confirmation and any required
student materials.
Typically the confirmation and
materials are sent to the student about 3 weeks prior to the class
date. Students that show up at a
class location without
pre-registering, making payment and receiving the written enrollment
confirmation will need to reschedule for
another class date.
We
recommend that students register for classes as early as
possible. Courses
marked as "Closed" are full and no additional
students can be enrolled. Course
schedules and class availability do change frequently and
may change at anytime without notice.
The possibility does
exist that
a class may close before your registration with
payment reaches the LifeLink office. If this occurs you
will
be notified so that you may transfer to another class. To
check whether a class still has openings, contact the LifeLink office.
FEES, TRANSFERS CANCELLATIONS AND NO-SHOWS:
All course
registrations, course transfer requests (a request to transfer
a student from one course date to another), & cancellations
must be
received 4 or more days prior to the first day of a course. Course transfer requests received 4 or more
days prior to the first
day of a course will be processed at no additional charge.
Course transfer requests received 3 or less days prior to the
first day of a
course are subject to an additional $20 registration fee.
Cancellations received 4 or more days prior to the first day
of a course will
be processed at no additional charge. Cancellations received 3 or less days prior to
the first day of a course will forfeit one-half of
their
course fees and one-half of the course fees may be refunded. Cancellations
without notice (“no-shows”) forfeit all course fees &
will not
receive a refund.
